Transition information
Transition services means a coordinated set of activities for a pupil with educational disabilities that promotes meaningful, student-centered movement from school to post-secondary activities, including post-secondary schooling,training, independent living, and community participation.
It is important to remember that transition is not a curriculum, but a process that assists students in their journey through high school and onto post-secondary pursuits. Under current regulations the process starts at age 14. The goal of the transition process is a seamless, continuous delivery of services that allows the student to recieve necessary assistance for post-secondary success.
Listed below are some guidelines for transition planning.
During 7th and 8th grade:
Identify career interests
Develop social skills and functional living skills
Learn about your specific disability and how it effects you
Learn about transition options in high school
During 9th grade
Continue your career search
Identify post secondary school options and their requirements
Develop self advocacy skills
Develop a transition plan
Develop a vocational portfolio
During 10th grade
Update transition plan and vocational portfolio
Continue career and post-secondary option research
Continue developing self-advocacy skills
During 11th grade
Update transition and vocational plan
Visit colleges and technical schools
Take the SAT/ACT (apply for testing accommodations if needed)
Participate in job experiences - volunteer, paid and internships
Continue developing self-advocacy skills
During 12th grade
Upate transition plan and vocational portfolio
Make decisions about post-secondary plans
Complete applications
Contact community sources that may be of assistance
Collect documentation of disability
Complete admissions tests and entrance requirements
Contact support personnel at post-secondary option
Research employment opportunities

Work Program
The Center School in an attempt to help its students meet their transitional goals offers a structured learning experience. In-school, after school and off site work opportunities are available. Students may take advantage of paid and unpaid positions to gain workplace skills. These programs endeavor to develop the necessary skills to find, explore and keep a job.
