Two up-to-date copies of the BSA Medical Form and two copies of the front and back of the Scout's insurance card are required to attend summer camp. The forms must be notorized. Starting in March Troop 714 will have several notaries at Troop meetings for your convenience. Please make an appointment for a physical with your personal physician now or plan to attend the BSA Physical tentatively set for May 8th. Plan to arrive early if you wish to be seen. More information will be given as it comes available. Troop Permission Slip Due June 1 to Mrs. Hunt Required of all Scouts. Please return to Mrs. Hunt by June 1. Locker Combination This is a required form. The lock is for the foot locker that your Scout's clothes and individual items will be packed and stored in (see Packing and Equipment Checklist tab). Combination locks are preferred locks to use at camp. Have your Scout practice putting the lock on and taking it off the actual locker that he will be using at camp before he leaves for camp. |
