“Active Participant”

ü       When you successfully participate in AT LEAST 5 of the following activities and return this completed form to the Chapter President, you will have an asterisk added to your name in the coming year’s Chapter Yearbook. This will indicate that you are an “Active Participant” in activities benefiting the chapter and your own professional education. Please mark the items that apply and return

 Ø       Attended a state accredited music workshop, continuing ed. Class, Ashland, Or, or other Music workshop

 Ø       Present a Program for our Chapter or other chapters

 Ø       Participate or perform performance & conversations class

 Ø       Attended the Washington State or National Convention

 Ø       Entered students in the Chapter Student Recitals

 Ø       Took private lessons during the year with a certified teacher or college professor

 Ø       Entered students in WSMTA adjudications

 Ø       Entered students in the Spring Competition or other competitions, _______________

 Ø       Entered students in the Tri-City Music Festival (Sonatina or Ribbon), Young Artist of Distinction (Camerata), Music fest NW, etc.

 Ø       An officer, chairperson, or committee member in the state or local WSMTA
