This is a collection of documents that should be helpful for anyone undergoing transition. May I add that for anyone seriously considering transition and in need of advice the BIGGEST piece of advice anyone can give you is:

CHANGE YOUR NAME BY STATUTORY DECLARATION

The NHS and all other medical services will count your transition from the day you do this. Any time spent full time prior, including publications in your desired gender, or even public appearances will not count until this is complete. The changing of your name is a start date that is taken, meaning that, in general, no surgical processes can be allowed until at least TWO years after, nor any administrative services such as changing of birth certificates. However, administration of hormones can usually start three months after this date, or after at least 6 sessions of appropriate psychotherapy.
