The Toronto Central Home at Last Program ("HAL") helps seniors leaving hospitals to arrive home safely and comfortably.
Eligible seniors leaving hospital emergency departments or inpatient units are: 55 years or older, without support from friends or family upon discharge, and able to direct their own care. HAL is a partnership between hospitals, community support service agencies, and the Toronto Central CCAC. HAL is funded by the Toronto Central LHIN through Ontario's Aging at Home Strategy.

St. Christopher House (Lead Agency), Mid Toronto Community Services, Neighbourhood Link Support Services, SPRINT, St. Clair West Services for Seniors, Storefront Humber, West Toronto Services for Seniors, WoodGreen Community Services and Les Centres d'Accueil Heritage.
Acute Hospitals participating in HAL:
Mount Sinai Hospital, Princess Margaret Hospital, St. Joseph’s Health Centre, St. Michael’s Hospital, Sunnybrook Health Sciences Centre (incl. Holland Orthopaedic Centre), Toronto East General Hospital, Toronto General Hospital, and Toronto Western Hospital.
Sub-acute Hospitals participating in HAL:
TRI: Bickle Centre, TRI: Hillcrest Centre, TRI: University Site, Baycrest, Bridgepoint Healthcare, Providence Healthcare, Toronto Grace Health Centre and Westpark Healthcare Centre. |
