Adult Waiver/Parental Consent Form The Mentoring Group supports and adheres to the ESRB industry rating system for computer and video games and the voluntary enforcement of this rating system. Any person wanting to participate in the tournament must be 9 years old in order to be considered eligible. Any participant or spectator who is not at least 18 years of age is prohibited from playing or viewing "T" or (Teen) or "M" (Mature) rated software respectively without the signed consent of a parent/ legal guardian with proper ID on the day of the event or beforehand. This form must be filled out completely by the parent or legal guardian of the applicant indicated below. Proper identification is defined as (unexpired State Driver’s License, State ID, or Passport). If you are under the age of 18 and do not have written parental consent, you may not participate or attend this tournament. If you are under the age of twelve years old (12) you must have a signed parental consent form on file and are supervised by an adult 21 years or older who is solely responsible for you at this event.

PARENTAL CONSENT: BY SIGNING BELOW YOU UNDERSTAND THAT THE “T/TEEN” OR “M/MATURE” RATING OF CERTAIN GAMES IN THIS EVENT MAY NOT BE SUITABLE FOR CHILDREN LESS THAN 18 YEARS OF AGE. YOU ARE CONSENTING TO AND GIVE YOUR PERMISSION FOR YOUR CHILD/LEGAL WARD TO VIEW AND/OR PARTICIPATE IN THE MENTORING GROUP’S EVENT WITHOUT RESTRICTION OR LIMITATION. By my signature below, I grant my permission to allow the above named minor to view and participate in (T) and/or (M) rated video games. You also release any and all liabilities that may arise from your consent to your child/legal ward whom is a minor to participate in The Mentoring Group’s event. As the Adult, Parent/Legal Guardian you agree to assume responsibility for any items or property stolen, lost, or damaged and/or any minor or serious injury, whether accidental or intentional, to the above stated adult, child or legal ward during this event or in its surrounding vicinity. In the event that you or your child/legal ward may need medical attention, you authorize us to notify proper authorities and call an ambulance.
