1. Complete the Summary of TLC Learning ~ download this form to your computer, complete, and save.  You will be submitting this later in these instructions.
   
2. Gather student feedback from 5 to 10 of your students and/or parents on the affectiveness of your classroom site.  We would like you to create a survey assessment in your course shell to gather this feedback.
   
3. Prepare a word document contenting that includes the following:  
• survey results summary/reflection from your perspective
• course stats screen shot of student work or evidence of learning of your choice
• save as StudentFeedback
   
4. Submit the word document "StudentFeedback & Summary" to Learningpoint under Summary of TLC Learning Assignment
   
5. Complete the Participant Survey on LearningPoint  Using the Final Assignment in LearningPoint, attach #1: Powerpoint, 3# Summary Form; #6 Word document with survey results or summary and screen capture
