Goal: Develop Peer to Peer Mentorship Program
What makes our project unique is our talented and driven multidisciplinary team of consultants and navigators. Our Family Navigator is a peer liaison who works hand's on with families linking them to services and educational support opportunities that are identified by parents as the biggest need for their family. The FSP Project Assistant helps support community relationships so referrals are simple, user friendly and easily accessible to families. The FSP Project Case Manager helps the team develop an individualized family Road Map that is tailored to meet individual family needs. Our Family Strength Project Coordinator is what keeps our team strong! Together, we unite as one and make things happen as a team!
Project Coordinator Belinda Hawks -QMRP,BS, MPA
Belinda has over 25 years working in the field of mental health and developmental disabilities. Belinda will provide oversight to the work of the project,participate in monthly family planning meetings, facilitate community partner meetings, and co-facilitate 3L's group. Aimee has been working with persons with developmental disability and their families for over 6 years in a variety of different settings. My role for the team is to coordinate services with you and your family the easiest way possible. I will help make community connections with organizations specific to your needs, and work with your current support system in assisting you and your family attain their goals. Project AssistantTBD Family Peer NavigatorSamantha Gardner – Certified Peer Navigator Samantha is a certified peer navigator by Wayne State Developmental Disability Institute and mother to a 7 year old son with ASD. My role is to be a guide and mentor for the project team. I want to empower your family to achieve your self directed goals by "hands on" with your family. Together we can learn about the many possible opportunities for your family in our community. |
