1. A clear copy of the picture and information page of your passportThis can be scanned and e-mailed or faxed so long as the copy is clear. Our recommendation would be to include this form in the same mailing as the c5360 form to ensure the quality of the copy which we receive as customs will not accept this if it is not legible. 2. A clear copy of your Japanese VisaIf you have received your long term Japanese visa please forward a clear copy to us. If you have not yet received this item please do let us know. 3. A clear copy of your most recent entry stampThis is the stamp which you receive in your passport at the time of your entry into Japan. For our purposes this will be valid for 6 months however this must match the date which is stamped on the c5360 form. 4. The orignal c5360 form (IMPORTANT!)Unlike the other forms a clear copy will not be sufficient for this documents, we do require the original. The full name is the 'Unaccompanied Articles customs form c5360', you will receive a copy during your flight into Japan as this is required of all visitors however please note that for the purpose of customs clearance you will need to complete the form in duplicate. Kindly ask the flight attendant for a second copy accordingly. Please note that this may also be received in the baggage claim area before you pass through customs. You will need to hand both forms to the customs officer after picking up your luggage, they will stamp them and hand one back to you. It is this original stamped form which we will require from you. A clear copy of your visa from the country of originPlease note that this will only be require if you do not hold a passport from the country of origin. For example if you are a U.S. passport holder and your shipment is coming the the U.K. The reason being that we will be clearing your items as used household goods as opposed to a commercial shipment.
