| As per a September 8, 2005 letter sent by Wendy Matos Negrón, Ph D, Executive Director of the Medical Assistance Program of the Department of Health to the Regional Directors, Local Office Supervisors, and Certifiers of that same program, the procedure for homeschoolers applying for PR Medicaid (Reforma) is as follows: -The father, mother, or legal guardian will certify in writing that the minor(s) are homeschooled. -The certification should include the complete name of the minor(s), address, date, and signature of the father, mother, or legal guardian Should you have any problems applying for benefits, please feel free to contact us or visit our legal message forum to see if your problem was addressed there. |
