These are all of the forms you will need for ski rental and waivers at Silver Mountain. Please print off all of the forms below and bring them to the mountain with you.


You will pay Janelle for the waterpark and receive your gray band, do not print off Flow Rider forms to turn in to Janelle any more, but instead go to the front desk of the waterpark, fill out a form if it's the first time that you attend in each month and then each time that you enter during that month they will look up your form and issue you a red band for the Flow Rider.  Hope that all makes sense...

For the Lesson Liability Forms (Below), you need to print out and fill out a form for every person in your group under 18 who will take a lesson. Only needs to be filled out once a season.

For the Rental Waiver and Form (Below) a form needs to be filled out for every person renting that day. It is necessary to bring a new form every time you are renting. The Rental Waiver and Form must bear the adults signature and be handed in EVERY TIME you or your child rents. However, you may fill it out (don't sign it yet) photocopy the filled out form, THEN sign it and send the copies with your child each week.  That is the simplest way at this point.
