font i used Futurist fixed width and tube of choice

resize template to 600 x 600 pixels 

layer 4 and 5 paper 10 80%

white circle layer paper 11 80% 

black whole circle paper 14 80%

dots paper 9 80% 

layer 1 paper 28 80%

red circle layer paper 8 80%

black lines layer paper 7 80%

green circle layer paper 10 80%

wordart 1 80% 

skull head 1 and 2 100%

ribbon 1 80%

add your tube 

apply drop shadow to each layer 

merge and resize 90%

Deco 3 90%

Deco 1 and 2 40%

add your name and copyright 

save as a jpeg or png
