By submitting this request you agree to the following:

 

 

When using rooms or property of the Shiloh Baptist Church I will be responsible for:
 
  1. Advertising and making directional signage. (when necessary)
  2. Insuring that all flyers and posters are approved by the church office prior to mounting
  3. Removing all signage and supplies that relate to this activity
  4. Returning all items and furniture to their proper place
  5. Insuring that the floor is clear from paper, pens, pencils, and other visible debris
  6. Insuring that small waste cans are set outside of classroom if empting is necessary
  7. Insuring that large trash cans are emptied and trash bags are taken to the outside dumpster.
  8. Insuring that tables and chairs are set-up and properly stored away
  9. Insuring that desks and chairs are neatly arranged
  10. Turning off the lights
  11. Closing the door
 
PLEASE READ CARFULLY

     This is a room and equipment request form. We will do all that we can to accommodate your function; however this form does not guarantee availability. Please submit all requests no later than two weeks prior to your event.
     In the event that your request cannot be granted, you will be contacted by the facilities manager or designee to discuss alternatives and the finalization of your request.

Monday thru Friday  8:00am - 9:00PM
Saturday  8:00pm - 12:30pm
Sunday  1:30pm - 3:00pm
--Special considerations will be made on case by case basis--
