I have created a website for anyone to download and use as their own. This site is PHP/MySQL based with full CSS capabilities. This site can be used as a general product or service information site and it can also be used to collect sales leads.

When used as a lead collection website, the following fields are collected:  

  id - Automatically increments a unique number for each new lead
  fname - First Name
  lname - Last Name
  email - Email Address
  optin - If the lead want to opt-in to future email campaigns or not
  date - Date and time the lead signed up
  keyword - Keyword used to find your website
  groupid - Paid search group id
  campaigntype - Paid search campaign type
  pageid - The page the user used to fill in the lead collection
  referer - The refering page the lead came from
  ipaddress - The leads IP address
  product_id - Custom product ID which can be used in paid searches or product listings

You are welcome to add to this as well. I just ask that you provide me with the new code so everyone can enjoy it. I will give credit where credit is due.
