Dr. Zonca has closed Peak Family Medicine; however, he is still seeing patients on the Campus of Rose Medical Center in Denver, CO. 
This website will assist patients in communication with his new practice in case they did not receive the letter regarding closure or are unable to contact the new office.

Why Use Us!

Peak Family Medicine is no longer providing active care to patients.  We are in the process of closing down our business due to financial reasons.  The providers who took care of patients at Peak have decided to join Ascent Family Medicine.

Patient records


If you were a patient of Peak Family Medicine, your medical records were transferred for safekeeping to Ascent Family Medicine in order to comply with HIPAA security of your medical information.  You may contact them at 303-322-0212 as the Peak phone number will be disconnected.  If your pharmacy is trying to obtain refill authorization, please ensure they have the right fax number for refill of your prescriptions: 303-322-0208.
