A blank Attendance Sheet can be found in Google Documents. At the beginning of each year, and as a new member is added, a Junior Leader should fill this out with the members first and last name. Once printed out, this sheet should be kept in Judy's meeting clipboard so that it can be used at each meeting. At year-end, a Junior Leader can let each member know how many meetings they attended. A blank Emergency Contact list can be found in Google Documents. A Junior Leader should fill out this sheet by copying and pasting the project roster provided by the Club Community Leader. Multiple copies should be printed out and given to Judy. A blank copy of the 4-H Youth Medical Form can be found in Google Documents. Parents would need to fill these out and give them to Judy if the club travels to an event and the parent is not going to attend the event along with their child. A blank copy of the 4-H Dog Project Record Book can be found in Google Documents. Junior Leaders should print the first page for each member/dog taking the club's canine good citizen test as it provides a record of the dog's shots. Members should print this out for inclusion in their record book.
