Little League Canadian Volunteer Application for 2009 (french) Note: OLLB does not require the three references for purposes of this form. Every volunteer must complete one and deliver to the Safety Officer at 419 Cloverheath Cr., Orleans, ON, K1E 2R9 (turn right off of Tenth Line onto Des Epinettes, left on prestwick, left on Des Praires, right on 2nd Cloverheath Cr.) before he/she is allowed to start work. Please get it done early. Please note that without two proper ID's, found listed on the back of the police record check, the form cannot be processed. If you are going to hold the position of Safety Rep for your team, you must have a valid criminal check and have successfully completed the Health and Safety Certification Program.
