Required Forms for Tutors:

Tutoring at Oasis requires the completion of the "Archdiocese and Oasis Forms" listed in the boxes below. If you are a NEW tutor, please print and fill out each of the forms listed in the two boxes and turn them in at your orientation session or the first time that you come to Oasis. If you are a RETURNING tutor, print, complete and turn in the 2 "Oasis Forms" at your orientation session;  and make sure you have completed the "tutor information" form on line as soon as possible.
