Please consider joining our wonderful organization! Oakleaf membership is open to the following groups within the Navy Medical, Dental, Nurse, and Medical Service Corps living and stationed in the Jacksonville area:
1. Spouses of Active Duty officers
2. Active Duty officers
3. Spouses of retired Active Duty officers
Annual dues are $25 (check made out to Oakleaf Club of Naval Hospital Jacksonville), but from February 1 through May 31 of 2012 the membership will be half price ($12.50). If you would like to join Oakleaf, you have three options for getting your membership form in:
1. Fill out the below online form, and pay your $12.50 dues online through our secure PayPal system (see button below).
2. Print out the downloadable form at the bottom of this page and mail in a check (address is on the form).
3. Bring in your filled out form and check to an upcoming Oakleaf event and hand it to a board member.
