2011 Ballot set up with text fields and signature field for email submission to NMAA. See original email for list of candidates. Do not submit if you did not receive a ballot directly from NMAA.
This form MUST be completed to describe any red cards (ejections) shown, removal of spectators, significant injuries, etc.
Please also use this form to report particularly positive or negative events in your matches within the context of "Pursuing Victory with Honor".

The attached Misconduct Reporter application uses the basic functionality of my RefTrakr program to fill in the necessary information for you to file a misconduct report for the Arbiter. It uses the standard terminology for the NFHS misconducts and also uses the 7 + 7 suggested phrases for reporting the infraction.
Download it from this site, and extract the files in the compressing zip file to a location of your choice. Navigate to the location where you unzipped the files, and double click on the Setup file. Press "yes" or "OK" to everything that it asks you, and when it completes, you should have a new program item in your Start Menu. Click on the Misconduct Reporter menu item shortcut, and it will execute. This program has been tested on Windows XP only, but it should work on all 32-bit Windows operating systems (no warranty or fitness for a specific purpose is implied, blah, blah).
To use it, you fill in the home and visitor team names (if you don't, they default to "Home" and "Visitor"). Selections that refer to a "drop down list" have an arrow to the right of their box. Click on the arrow, and you can make your selections from the options offered in the drop down list. Select the team, time, player, etc. When you click on the arrow for card color, you can select yellow or red. Whatever you select will limit your misconduct types to that color. Select the type, i.e. DIS, USB, etc., and the suggested phrases will be available in the Description drop down list. Select which phrase you want, click on the description text box, and the selection will be transferred to the box. You can then edit it or add to it, either by adding additional descriptions from the drop down list or typing your own commentary.
When the description is complete, click on insert, and a formatted report of the misconduct will be inserted into the report text box. Do this for each misconduct in time sequential order, and each will be added to the report text box. When your report is complete, click "Copy to Clipboard", and you can then copy the entire report into the Arbiter comments area by putting the cursor in the comments area and pasting the clipboard into it (this can be done by selecting "paste" from the Edit option in your browser menu or holding the CTRL and v keys simultaneously).
