Students participating in a school sport must have a current sport's physical on file in the nurse's office before the first day of practice.  Sport's physicals are valid for one full year from the date of the physical and must not expire during the duration of the sport the student is involved in.  A complete physical such as for entrance to sixth grade will suffice as a sport's physical but a sport's physical will not be considered valid for the sixth grade physical.   Please click here for Sport's Physical Form:  http://www.d155.org/CLC/physicalcard.pdf
