The State of Illinois now requires dental examinations by a licensed dentist for students currently in the sixth grade.  The exam must be performed within eighteen months of the May 15, 2010 deadline, meaning that dental exams performed on your child between November 15, 2008 and the deadline would meet the requirement.  The completed State of Illinois form must be on file in the nurse's office by May 15, 2010.  If your student receives free or reduced lunches this requirement may be waived.  Watch this site for information regarding low cost dental clinic in our area!  Click here to download the form:  http://www.idph.state.il.us/HealthWellness/oralhlth/DentalExamProof.pdf
