If it is necessary for your child to take medications while at school, a Medication Authorization form must be on file for each medication in the Nurse's Office.  This form must be completed and signed by both the parent and the child's physician.  The physician must authorize the administration for prescription medications as well as over-the-counter medications such as Tylenol and Ibuprofen.  The medication must be sent to school in the original bottle.  No medications will be given to the student without proper documentation.

 

At the end of the school year, please remember to pick up your child's medications in the nurse's office.  Any unused medication left at school at the end of the school year will be properly disposed of.
