Dear Parents/Guardians:

 

Thank you for taking the time to fill out the Title VII Indian Education Student Eligibility Certification form. Specific information must be included to ensure eligibility for your child.

 

Directions for each highlighted section:

ü      Name of tribe, band, or group: Write in the name.

ü      Name of individual with membership: Write the name of the individual who is claiming to be a member of a tribe (Blood quantum is not necessary for enrollment in this program).

ü      Individual named is: Who is claiming tribal membership? Choose child, child’s parent, or child’s grandparent.

ü      Name and address of organization maintaining membership data for the tribe, band, group: Provide as much information as possible. List the city and state of the organization keeping paperwork or BIA information (if applicable). If your eligibility is being researched, write researching.

ü      Parent signature and date: Please sign and/or date the form.
