___________________ Registration Procedure:You can mail/email the registration forms. You can bring in your payments or be mailed and made out to the Milwaukee Home School Group, 2400 N Cramer St., Milwaukee, WI., 53211. You can pay in cash, check, or cashiers check. All checks can be made out to the Milwaukee Home School Group (MHSG). For returned checks there is a $40.00 processing fee. No refunds available. Payment Fees for Fall Classes and Lessons: All 55 minute Classes are $125.00 for nonmembers. 5 years - 8 years classes is $65.00 for nonmembers. For music lessons contact Gabe Hammer on Thursdays or email at gabroso23@hotmail.com. Membership and Discount: You get a FREE semester membership with any $125.00 class and with each additional $125.00 class you get $20.00 off the price. For any $65.00 class you get $15.00 off a semester membership. Members get $7.00 off any $65.00 class. Membership is $35.00 each for fall / Spring semester and summer membership is $20.00 Registration Forms: I have the registration forms for the fall classes as attachments on this page or you can email me for the forms. This is the list of registration forms needed to be filled out; Registration form (one per child), Medical Liability Form (one per Family), a policy form (one per family), and a school age agreement form for children 10 years and older. Attendance/Payment Policies: For all the fall classes the fee is for the semester not per class. Any missed classes will not be reimbursed. There will be only make up classes for weather and if the teacher would miss a class. Then the make up date will be for the whole class and not individually arranged. No refunds.
