Please complete the intake form above BEFORE emailing staff listed below. The form only needs to be completed ONCE. This form goes only to YOUR GAE Staff person and to his/her Administrative Assistant at GAE headquarters. You do NOT need to print form or email it. By clicking on the "submit" button at the bottom of the form, you will automatically send it tot he recipients listed above.

GENERAL INSTRUCTIONS:
1. Do NOT resign!
2. Put your first & last name and preferred personal phone number on ALL faxes & emails.
3. FAX documents you receive from school management to 678-302-7524. (You may scan and email these documents if you prefer.)
4. Email your draft response to these documents at barbara.kirby@gae.org. Do NOT fax your responses - send by email.
5. Call Barbara Kirby at 678-837-1121 if you have any questions.  
6. This form will remain confidential and will only be seen by your GAE UniServ Director and/or other essential GAE personnel in order to process your intake form and assist you.

FREQUENTLY ASKED QUESTIONS

1.  Do NOT resign or sign a separation form until you speak with a GAE representative. 

2.  If you are asked to attend a meeting which you are uncomfortable attending on short notice, ask administration if you can reschedule for a later date.  If they say no, ask to bring someone in with you to the meeting for support.   If they still say no, bring a notepad and pen with you to take notes.  Let administration know that since you had short notice and don't know what the topic of the meeting is, you'll need to take notes and respond at a later date to ensure that the information you provide to them is accurate. If you are directed to answer any question during the meeting, a possible answer can be, "This matter was just brought to my attention, so I'm not familiar with the details.  I need some time to locate documentation in order to answer your questions".  If you refuse to go to the meeting, you could be charged with insubordination. 

3.  If you are asked by an administrator or SRO officer to write a statement based on accusations made against you of violation of the code of ethics, or have been arrested or charged, do not make further statements without calling GAE legal at 678-837-1100 immediately.  If you are told to fill out a statement or else you will be insubordinate, clearly write in your statement that you are doing so under duress.  DO NOT SPECULATE OR GUESS IN YOUR STATEMENT as to what you have been accused of doing.  A possible response can be:

“On ____ day of ________________, 2011, I was ordered and directed under threat of disciplinary action to submit this statement by [name/title of administrator].  Therefore, I submit the attached statement [or I will attend the meeting] only at the direction of my employer.  I also understand that the school system requires this statement [or my verbal statement made at the meeting] for its internal use only and will not release it to any other agency."

4.  Do NOT conduct your personal business on school computers. Here is why.

5.  NOTE: The deadlines for contracts is MAY 15 again this year! That means May 20th is the last date for notification allowing for the mail to run. Those of                        you waiting for April 20 to take some form of action should push this back until May 20. Change was made in mid-March 2010.
