This site will allow staff members to access the Martin High School Student Assistance Team (SAT) forms.

Please remember to contact the principal or counselor prior to referring a student.

 

Form descriptions

The SAT Information form outlines the process and gives descriptions of SAT issues in a written format.

The Martin Middle High School process form outlines the process in a flow chart.

The Martin Student Concern forms (teacher and parent) forms are designed to be completed by the respective staff members or parents and are needed for all referrals. 

The File Review form will be completed by the counselor and/or psychologist after the SAT schedule has been approved by the principal.

The Student Intervention Assisting Team (SAT) Meeting Recommendations form will be completed at the meeting and used to document the ongoing intervention process.

SAT Meeting Recommendations Form




The current Scheduled dates for the 09/10 year.  Meetings will be held on the same day as the Elementary and we will have a half-day sub each day to allow the presenting teacher to attend the meetings.  The April 29th meeting should be for reviewing plans in progress unless their are extenuating circumstances.
