Staff utilize these three-ply forms to make a student aware when they are not following the behavioral expectations. Staff discusses the incident with the student to be sure that they understand what they were doing incorrectly. It is important to emphasize the school expectations when discussing the incident report with the student. A yellow copy of this information sheet may go home with the child for parent signature.
