* A short paragraph or two on why you want to participate.

* What you hope to get out of this, learn, take away.
* What you believe you will contribute, add, give to the program, process, other participants.

* A check off box (or signature) that says you are committed and able to attend all nine sessions – or at

   least seven or eight, committed and able to complete the homework between sessions, etc…
