PDS Activities Record (PAR) Form - Download

Use the attached PDS Activities Record Form (PAR) below to record your professional development activities. Right click on the link and select "save to target" to download. The file is a formatted spreadsheet which will automatically calculate the points for each type of activities you enter. You are advised to make a record everytime you complete an activity. Once you enter sufficient activity records to qualify for the PDS award, a status bar "ACHIEVED" will appear at the end of the spreadsheet. You can then print this out, sign the declaration and send this and the softcopy to the PDS Board Secretary when invitation for application is announced.
 
Filling details in the form
 
The form will only allow you to fill in the boxes in yellow. For dates, select from the pull down boxes. Ensure that you provide sufficient details in filling in each item so that it can be examined and verified by the PDS Board or other members.   

1.   Follow the convention below when filling in each activity in the form

    Format:
    <Name of activity> / Conducted by <person/company> / Organized by <organization> /at <location, Country (if not in Singapore)
 
    Example:
  • 2008 LAS Conference / Organized by LAS / at Singapore Polytechnic
  • Future of Knowledge Sharing / Conducted by David Gurteen / Organized by NLB Academy / at NLB
  • Integrated Information Management and Access - New chances for Museums, archives and libraries / conducted by Dr Martin Doerr / Organized by Heritage Conservation Centre, National Archives / at Oldham Lecture Theatre, NAS
  • Visit to NIE Library / Organized by LAS Art Libraries Group
2.  You may use any citation style to enter the details of your publications
 
3.  Name your file as PDS-2010<yourname>
