Instructions: Your “Senior Information Sheet” will be used to write letters of recommendation for your college and scholarship applications. Make sure your answers are accurate and as detailed as possible.  Download the attachment below.

Please note:

1.       Student must make appointment to request letter of recommendation from Counselor and/or Teacher.

2.       Not every college requires a Letter of Recommendation. Only request if it is required.

3.       Submit request to Counselor and/or Teachers at least two weeks before due date. More lead time is helpful.
