Everyone attending camp must have a Day Camp Medical form on file.
This includes Scouts, parent volunteers/chaperones, siblings in the Tot Lot, Jr Staff and Staff. You can download the form using the link at the bottom of the page. We will also have some forms available at Roundtable. ALL camping events through the BSA, including Day Camps, are using one standard Medical Form. This standard form includes multiple sections. Any camping event that does not involve an overnight stay of greater than 72 hours does not require a doctor's check-up or signature. Day Camp requires all participants to submit Medical Form sections including health summary and parent/guardian release signatures. We require only Parts A & B. (We do not need part C which requires a physician's signature. ) The medical form is available to download below. It is NOT necessary to print all pages of the medical form. The required sections can be printed on the front and back of one sheet of paper. If printed or copied on multiple pages, please be sure the participant's name is on all pages and that only one participant's pages are stapled together.
Please make every attempt to turn-in all youth medical forms (completed and signed) by the June 6th meeting. Forms for any adults who are scheduled to attend camp may be turned in then as well. It is vitally important that camp staff is aware of serious conditions in advance of camp. It takes time to process the 300+ forms and it is important that camp medical personnel be aware of these conditions before the participants' arrival in camp. Thank you for your assistance. All medical forms are kept in a secure medical box and access is limited to top camp staff, your Pack's leaders and the medical personnel.
The form does require a parent/guardian signature for emergency medical treatment and a signature for permission for photos/videos. Please make sure the emergency contact information and phone numbers are current.
Please be accurate when completing the forms.
Immunizations should be up to date and listed in the space allotted on the form.
Please do not attach copies of other medical forms, including immunization records.
Please turn in medical forms before camp begins, especially for all youth.
Medical forms will be destroyed at the end of camp if unit leader does not pick them up. Medication and Emergent Event Procedures
• The Camp Medical Officer must administer all medications. All medications must be clearly marked with the patient’s name in the original container. • Inhalers and Epi-pens are to be kept with the participant. Persons with Epi-pens will be asked to wear a special wristband to identify their medication. • If a child has a serious problem that the Health Officer cannot manage fully on site, the child will be sent to St. Margaret’s Hospital or Children’s Hospital. • Please inform your adult leaders of any situation (medical or emotional) of which they may need to be aware for each participant. |
