Referee Self Assign

INSTRUCTIONS
  • -Officials must be USSF registered with a current badge.
  • -Officials must be registered with the HSA from the web site.
  • -Officials may only selfassign themselves two(2) games per day.
  • -Officials must be two(2) years older than the group they are refing.
  • -Officials must be at the field a minimum of 15 minutes before games begin.
  • -Officials must notify HSA 3 days prior to needing to "turnback" an assignment. If unable to make an assignment after the 3 days, it is your responsibility to find a qualified substitute.
  • -Confirmation of assignment will be made on this form. Refer to column at the right for confirmation.
