There are several forms that need to be completed for your student at the beginning of each school year. For your convenience, these forms are attached below. Hit the download button to view each form. You can print these forms and fill them out by hand, or you can type on the computer and then print them out. Remember that you probably cannot save your typed work, so print each page as you complete it on the computer.
To speed up the registration process, you can complete these forms and bring them with you to student registration.

NEW STUDENT APPLICATION FOR ENROLLMENT FORM:
This form must be completed for all students in grades 7 to 12 who are new this year to the Gallatin School District.
RETURNING STUDENT ENROLLMENT FORM:
This form must be completed for all returning GHS students.
NEW STUDENT TECHNOLOGY AGREEMENT:
This form should be filled out for all students who will be in 7th grade and for all students who are new this year in the Gallatin School District. Please read the document. There are two signature pages at the end of the document that must be signed and turned in at registration.
RETURNING STUDENT TECHNOLOGY AGREEMENT:
This one-page form is for all students in grades 8 to 12 who attended Gallatin High School last year. Student signature is required and form must be turned in at registration. Your signature on this form simply implies that you will continue to follow our technology guidelines.
ACCESS TO STUDENT GRADES:
All parents are encouraged to complete this form. Whether you are already accessing your student's grades or if you want to sign up for the first time, this form needs to be completed. This is also the form that is needed to sign up to receive the Daily Bulletin via your email account.
PARKING PERMIT:
All students who plan to drive a vehicle to school must complete this form.
SPORTS PHYSICAL FORM:
All students who plan to participate in any high school athletic sport must complete this form prior to participation in the sport. This form needs to be printed off and taken to a physician.
FREE & REDUCED LUNCH FORM:
This form must be completed every year for families that qualify for free or reduced lunches. If you think you qualify for free or reduced lunches, then complete one form per family.
KIDGUARD INSURANCE ENROLLMENT FORM:
This student accident/health insurance is optional. Please read the brochure to see if it is something you want to purchase. You do not bring this form back to school. You must mail this form to the specified address.
