Tena koutou katoa, Talofa lava, Malo e lelei, welcome.
We are pleased you have found this site and hope you will visit frequently throughout the period of the primary care collaborative. We will use this site to:
- provide an overview about the collaborative
- make available electronic copies of key resources
- facilitate the sharing of stories and learnings from practice teams
- provide news and announcements
Reminder.....
While the formal stage of the collaborative has finished, keep using the resources on this website, doing PDSAs and making small, regular changes towards continual quality improvement.
Go back to your ACIC and PACIC questionnaires, look at what areas you are doing well and what areas could do with improvement. Use this plus your population audit tool data to guide ongoing improvement work.
This site will be re-used and modified for a new Collaborative project starting end of Sept 2010 focussing on refugee health. So stay posted, for more information and resources in the near future!
