Created draft instrument to use with teachers in a K-12 school district. Should I put into a GoogleDocs form? I want teachers to fill out single page (two sides) in a staff meeting or workshop for higher response rate. Not sure I can get a high response rate if it is online (fill it out when you have time...). I think I will create a form, but use it for inputting data from paper forms.
