Pizza Order forms due Jan 22

·         Payment for any fundraiser should be by check (NOT CASH).

·         Checks to be made out to: Ephrata Band Parents Association (EBPA)

·         Payments need to be turned in when returning order for that fundraiser.

·         Place all orders & payments in a sealed envelope with band member name and name of fundraiser on outside.  Return orders/payment to Mr. Goss at the Middle School.
