**for parents & students **

Each student, both incoming and returning, must submit all required health forms to the Health & Wellness Center no later than July 31, 2010.  No student will be allowed to participate in preseason athletics and/or be admitted to classes in September if these forms have not been completed and returned to the Health Center.

Every student must have a physical examination performed and documented by their healthcare provider within the last year.

A nurse is on call 24 hours a day

when school in session.

The Health Center is  located in the basement of Hyphen Hall, next door to the laundry.  The entrance to the Health Center is down the stairs from the locker area in 2E cafe. ( There is a sign on the door).  Mary Moore's office is located at the end of the Health Center hallway, on the right side.
