If you would like to get started with an Individualized Education Plan, please do the following steps: 1) Take the IEP Survey at https://delaware.qualtrics.com/SE/?SID=SV_8vvY5Z0w8BoRtdy 2) Email Nicole for an IEP appointment time. 3) Come ON TIME for your appointment. If you miss your appointment TWICE, you will not be allowed to make a third appointment time. 4) Print the IEP work table below. Fill it in EVERY time you come to the SALC. 5) Have an SALC staff member check your work table one time per week. 6) At the end of the session, present this information to the SALC and to your teacher. |
