1. Log in to the Google Docs.

2. Select New/Form in the Google Docs. The default form will be displayed.


3. Edit this form. Name is already there as a first question. You can edit second questionas email for example (yellow block). Click here to see.


4. Add question by clicking + sign at the top left of the form. Edit the question title as done before, but question type should be "Paragraph text" (click drop downbox). Click here to see.


5. Once the form is complete, then go to google site.

6. Create new page for Feedback and edit the page.

7. Insert the spearsheed form and select the form that you created in Google Docs. You are finished.
