Application Process (due Jan 29)

Complete the following forms and return to Dr. Peter Cardon by January 29. You can bring the applications to him in room 112-C of the Carolina Coliseum. If by chance he is not in, you can leave application materials with the administrative assistant, Ms. Barbara Rice, in room 120-A.
 
  1. USC Departmental Program Application
  2. Direct Deposit Form (use the attached .pdf file named "Dominican Republic Maymester 2009" at the bottom of this webpage).
  3. Medical Self Assessment Form
  4. Copy of International Student Identity Card (required for emergency medical coverage)
