Below are forms which PTO volunteers may need to use during the school year. CHAIR DOCUMENTS CLASSROOM DOCUMENTS FINANCIAL DOCUMENTS - Tax exempt form : *Chairs should have received the Countryside Tax Exempt form with your chair packet. Some businesses require payment to come from the Countryside PTO in order to waive the sales tax (they won't accept a personal check along with showing the copy of the tax form). Also note that "prepared food" such as boxed cookies are not tax-exempt.
At the following businesses, the Countryside PTO has set up tax exempt accounts for business expenses and Carnival raffle prize purchases: Apple Store Southdale, Office Max, Staples, Best Buy
