At the bottom of this page are links to download the 2012 Copper Country Curling Club Membership Application, and the Minor Participant Release Form. Both forms are PDF files with several options for filling them out.

Because of the late start this year, we need payment AS SOON AS POSSIBLE. Please pay no later than January 15. Also, get your membership forms back AS SOON AS POSSIBLE. We are scheduled to start league games on January 17. That means I need to put teams together by January 15, to let everyone know when to show up, and who they will be playing with. Your cooperation with this will be greatly appreciated.

FIRST

If you are feeling in a low tech mood, simply print the form off, fill it in by hand and bring it with you to the club or mail it to the club president at

You can fill out the form on your computer and email it to the club president using the button at the top of the form or send to president@coppercountrycurlingclub.com. Don't forget to digitally sign the second page (or you can print that page out, sign it by hand, and bring it with you later.) If you fill out the form on your computer, it will require you to fill in all of the fields at the top of the form. The phone number field will require you to enter the area code and number (NUMBERS ONLY, no spaces or other characters.) You can always print off the form so you have a record for yourself.

Don't forget to note which night(s) you wish to curl, and use the "Comment" box to indicate any special information you might want to include, such as team preferences.

PAYMENT OPTIONS

As in the past you can pay by check or cash. New this year is to pay via PayPal.

PayPal

On the form beneath the "TOTAL" is a button with the familiar PayPal "Buy Now".

1) Pressing that button will take you directly to the PayPal login page on your browser, and a form to submit payment for "Copper Country Curling Club Membership" will be shown.

2) Enter the amount you need to pay on the left half of the page, and press "Update".

3) IF you have a PayPal account already, simply login.

If you do not have a PayPal account and would like to open one, you have that option.

4) Then press "PayNow" and you are done. Be sure to print a receipt for your records.

Cash

If you pay by cash, please put the money in an envelope with your name on the outside or enclose your membership form with the cash. DO NOT HAND US CASH and expect us to remember who gave it to us. The majority of all dues issues over the past few years have come from that exact action.

Members under the age of 18

If you are under 18 years of age, you MUST have a waiver form signed by your parent or guardian. Download the Minor Participant Release form, fill in all fields and have a parent or guardian either print off, hand-sign, and mail to the address above, or digitally sign and email to president@coppercountrycurlingclub.com
