8th grade students. In order to continue using your digital locker, please return a signed copy of this form to your teacher.

Google Apps for Your Domain: Education Edition Agreement of Responsibility

Along with opportunity comes responsibility. Please sign the Google Apps agreement of responsibility below to indicate that you
understand the responsibility associated with this learning opportunity.

In order to comply with federal law and the Google Terms of Service, all students 13 and under are required to use an account
owned by a parent or teacher with parent or teacher supervision.

School responsibility

The Google Apps accounts assigned to your child were created by the school and assigned to your child as a digital locker. The
account number corresponds with their physical locker number. No student personal information was collected by Google in
creating these accounts. As per school policies, all activities requiring Internet access are supervised by the teacher. The school
controls which Google services it gives students access to. At present, email, chat, and start pages are turned off for students.
Internet safety education is included as part of introducing new technology tools. Teacher supervision, school filter, and spot
checking student accounts will be used to ensure that students use of digital tools follows school policy.

I will use these tools for school related purposes and follow all school guidelines when using these digital tool. I will report any
evidence of cyberbullying to a school administrator, guidance counselor, or teacher. I will also follow my parent or guardian
guidelines for use of Internet tools in and out of school.

Parent/Guardian responsibility

I understand that supervision of Internet use outside of school is my responsibility. I will discuss our family rules and
expectations for using Internet based tools, including Google Apps Education Edition, with my child and accept responsibility for
supervising my child’s Internet access outside of school. I will report any evidence of cyberbullying or other inappropriate use to
the school and can ask that my child’s account be terminated by contacting a school administrator followed up with a written
request.
