BP CDS-QI at RCH

Blood Pressure Control

TMIT Consulting, LLC was funded by the California HealthCare Foundation to help Redwood Community Health Coalition (RCHC) achieve its objective to have participating RCH community health centers (CHCs) improve the percent of patients that have their BP controlled (i.e., to <140/90).

The overarching RCH goal for the project is to 'get better faster - together' by functioning as a coordinated system (realizing enhanced efficiencies and effectiveness) in their efforts to improve care delivery, outcomes and experience for all participants. The BP QI work and tools are being scaled to other improvement imperatives at RCH, as well as CDS/QI efforts in other organizations.
