Brown School Swim Team
The KHSAA requires each athlete to submit the following – which can be found in the 09 – 10 Student / Parent Handbook for Brown School Athletics. Any forms not turned in will delay your start for swim team participation. o Parent Permission/Release – High School Athletics form – this must be completed by your parent and must be accompanied by cash or a check for the amount of $20.00 payable to The Brown School. This insurance is good for the current school year. There are additional forms which must also be completed and submitted: o Student Athlete Information o Pledge Sheet o Penalties for Violation of Parent Code of Conduct o School Volunteer Records Check Students must have a physical examination o Sports Physicals – this must be completed by a physician and is good for 1 calendar year. Brown School Swim Team Swimmer Information Tentative meet schedule for the 2009 – 2010 school year:
We may add a few more meets, depending on availability, time and the weather! On meet days there will be no practice. Swimmers who will require rides to swim meets will know ahead of time so we can make arrangements.

Swimmer Information YMCA membership will need to be purchased for November 1st through February 5th. Swim Team goes from November 1st to Regional’s, February 5th and 6th. State Finals are February 12th and 13th, if you qualify.
The swim team will be practicing at the downtown YMCA. In order to do this, all swimmers who practice with the team must purchase a Community-Wide Youth membership form the YMCA.
The YMCA requires: o an initial registration fee of $30.00, as well as a monthly fee of $32.00. o the YMCA requires an electronic funds transfer payable by direct debit from a checking or savings account, or by credit card, drafted the 1st of every month. o at the conclusion of the season the YMCA will need to be notified 15 days in advance, to cancel the membership or they will continue to take out monthly drafts. o A parent must sign the membership application for it to be valid.
Practices will be: 4:00 -5:15 starting Monday November 2, 2009 We will go Monday, Tuesday, Thursday and then adjust for what we need accordingly. If needed, we will switch one of the days for Friday if this works better.
Meet in room 315 after school. This is a good time to spend about 45 minutes on homework! As a group we will to walk over to the YMCA.
