HINI Virus Vaccinations: 

All of the Parent information and permission forms were sent home November 10th.

Please remember to complete the from in its entirety and return it to school by Friday November 13th as there is much to be done on our part to plan these vaccination clinics and therefore we need the forms returned to us ASAP. 

We need the vaccine form sent back even if you answered "NO" to the vaccine.
