Paperwork Nights
 
We will be collecting paperwork on Wednesday, July 15th and Wednesday, July 29th from 6 - 7:30 at the concession stand at the Bellingham High School Football field.  Per national Pop Warner rules, your child will not be allowed to start practice or get football equipment until you have submitted all the required forms.  We are collecting the following:
  1. Original birth certificate for all new participants.  If your child was rostered for the entire 2008 season, we have a copy already.
  2. Physical exam dated after 01/01/2009.  You can provide this in one of three ways:  the 2009 Pop Warner Physical Fitness and Medical History Form Part II, a copy of the standard form issued by the physician's office, or a note stating that your child is "physically fit and can participate in Pop warner football or cheerleading".
  3. A legible copy of the entire report card for the 2008-2009 school year with all terms listed.
