Hello Everyone!

 

In an effort to centralize all documents in a form that need not be reformatted, we have opened an online documents database.  Please contact Connie Li (cli@jcyc.org) if you notice that a document should be added or if an additional format of a document is needed.

 

To clarify, Monthly Performance Reports should be submitted by the 12th of each month with the following: a cost report, payroll, the performance report, personnel logs, single service logs, sign in sheets, all registration forms, pre-tests, post-tests, and client satisfaction surveys.  These documents are available on the tabs to your left (under "Reporting").

 

Also, every individual who is a part of a group, be it an environmental strategy group or an educational curriculum group, must be registered. All other participants -- Information Dissemination recipients -- should be given a client satisfaction survey.
