This form is filled out by a person acting in a role as the corporation's financial provider or senior officer (e.g., a CFO, if a small business). So why does it want personal information about them? The role might be the same next year, but there's no guarantee that it will be the same person - does one CPA have to call up another CPA to ask their favorite toy? And why is there such an emphasis on childhood experiences - this seems to be a psychodynamic therapy session as much as a tax filing session.

She’s on the Franchise Tax Board’s Top 250 Debtors List, whose members owe the state almost $184 million between them. Assemblyman Henry Perea, D-Fresno, used this list to help promote his bill, AB 1424, which would give the Franchise Tax Board new tools in going after these debtors, including suspending professional licenses, the ability to work with the IRS across state lines, and barring companies they lead from contracting with the state.
